How to get started
- Call the office to schedule your confidential consultation with Carolyn Zaumeyer, MSN, APRN (954.791.4498). Together we can determine if Bio-Identical Hormone Replacement Therapy is for you.
- Complete your New Patient Form (link below)
- Share your blood work with us, so that we can to check your hormone levels, as well as your CBC, metabolic panel, thyroid hormones, and Vitamin D3, and B-12.
- If you do not have current blood work, we have a laboratory in our office to run these tests, or we can send you a prescription to have your blood drawn at the laboratory of your preference.
- Return to the office for treatment.
